Step 1 - Referrals

Three Minutes of Administrative Time
Insurance Company

  • Name and Address
  • Adjustor name, phone and fax
  • Nurse case manager name, phone, fax and company

 

Referring Physician

  • Name, address, phone and fax
  • Procedure requested and
    rule out

 

Patient

  • Name, address, phone (H) and (W), and claim number
  • Date of injury, Date of birth, Social Security number and employer name

 

Step 2 - Scheduling
First:
An OCM agent calls the patient within 24 hours of the referral to schedule a convenient time and location for the test.
Second:
Once the injured worker is available by phone, OCM schedules and confirms the appointment with the patient.
Third:
OCM then e-mails the physician, and/or nurse case manager with the specifics of the test.
Step 3 - Follow Up

Medical Reports:

These reports are transferred electronically in the form of an e-mail
attachment within 48-72 hours of completion.

Billing

The authorized provider bills OCM once the service is rendered.

MRI

The HCFA is first reviewed for accuracy. Once determined, an OCM HCFA
and/or invoice is created and billed to the payor with an additional copy
of the medical report.

EMG

Upon receipt of the HCFA, the HCFA and medical report undergo a stringent
medical review process by an EMG board-certified physician.
